Navigating Change Objectives
This is a project funded by the Eastern Locality Partnership’s Health Inequalities fund. Drawing on the range of expertise within the partnership, we co-produced a resource to help families and schools support children making the transition from Primary to Secondary school.
The collation, publication and dissemination of the Navigating Change resource will mean that:
- CYP, parents, caregivers, practitioners, and school staff in eastern Devon will have easier access to a range of resources to support well-being during the transition from primary to secondary,
- The resources have been co-produced with families with lived experience and organisations that support them
- School staff, practitioners and caregivers will have more choice and knowledge of the support available, from both statutory and voluntary sector organisations in their area, and strategies to use when supporting CYP to make the transition
Medium term
- CYP, parents, caregivers, practitioners & school staff will feel more informed, confident and able to support CYP to ensure that the transition from primary to secondary school is a more manageable experience
Longer term
- The resource will contribute to county-wide work to reduce young person and caregiver anxiety in relation to transition, so that families report a more manageable experience, and attendance is improved.
